The Health Buddy Desktop, a web-based patient management tool, offers trends, risk stratification, and the ability to efficiently communicate historical patient information for large groups of patients.
The patient data, gathered by the Health Buddy Appliance, is transmitted to a secure data center where it is available for review using the Health Buddy Desktop interface. The applications quickly risk-stratifies and presents patient results, giving proactive providers the opportunity to intervene before the patient’s condition becomes acute. Patient responses are color-coded by risk level as High (red), Moderate (yellow) and Low (green) based on symptoms, patient behaviors and self-care knowledge.
The Health Buddy Desktop also allows care managers to generate custom messages for patients.* The message of 150 characters or less — which can include positive reinforcement for healthy practices, an appointment reminder or even a birthday acknowledgement — can be created using free style text. Patients will see the message the next time they log on.
