Health Management Programs - Health Buddy System

Using evidence-based telehealth to achieve comprehensive health management

The Bosch Health Buddy System uses telehealth to help patients manage a broad range of chronic illnesses and serious mental illness. This unique offering allows the care team to focus on the patient in their entirety when there are co-morbid health and mental health conditions, as opposed to treating solitary diseases. In addition to chronic care management, Bosch telehealth programs include post-acute programs for the recovery process and programs for serious mental illness.

The Health Buddy's health management programs utilize standard practice guidelines to focus on monitoring of signs and symptoms, promoting self-management, facilitating education and providing patient guidance. Each program is designed to collect standard outcome measures, including healthcare resource utilization, patient satisfaction, quality of life, health management compliance and patient population reporting. Many programs are also written with the caregiver in mind to enable Bosch telehealth systems to monitor their patient or family member while providing caregiver support.
