The Health Buddy's health management programs utilize standard practice guidelines to focus on monitoring of signs and symptoms, promoting self-management, facilitating education and providing patient guidance. Each program is designed to collect standard outcome measures, including healthcare resource utilization, patient satisfaction, quality of life, health management compliance and patient population reporting. Many programs are also written with the caregiver in mind to enable Bosch telehealth systems to monitor their patient or family member while providing caregiver support.
Our 60-day programs are designed to:
• Help prevent rehospitalizations within the first 30 days by focusing on vital sign and associated symptom management
• Assist with the care transition process by introducing content on health management behaviors and basic health education as the patient continues to stabilize and improve
