Using evidence-based telehealth for post-acute health management

The Bosch T400 Telehealth System helps patients manage post-acute recovery and avoid unnecessary hospitalizations. The T400 Telehealth System’s clinical programs include a variety of medical and serious mental illnesses and conditions along with 16 co-morbid and 4 tri-morbid conditions. This unique offering allows the care team to focus on the patient in their entirety during the post-acute recovery phase through care transitions and into chronic care management when needed.

The T400 Telehealth System clinical programs utilize standard practice guidelines to focus on monitoring of signs and symptoms, promoting self-management, facilitating education and providing patient guidance. Each program is designed to collect standard outcome measures, including patient satisfaction and quality of life.
